Please note the Dress Code: Proper golf attire is required. No blue jeans, cargo shorts or sneakers are permitted. All shirts must be collared.
If you wish to pay by check, submit the form below and make a check payable for the appropriate amount ($500 per golfer) to JAHMA Foundation and mail to JAHMA Foundation, 535 Route 38 East, Suite 123 J, Cherry Hill, NJ 08002.
